Seminole Lodge Candidate Registration

This is the candidate registration form for Seminole Lodge. Once elected by a local Scout unit, candidates must participate in the Ordeal to become a member of the Lodge. In addition to providing registration information and paying the required fees, you must bring a copy of your current BSA Annual Medical and Health Report to the event. A copy is strongly recommended. Note that unclaimed health forms will be destroyed after each event.
